MEDICAL and HOSPITAL

You have a choice of PPO Coverage or an individualized indemnity program. Other benefits include maternity coverage, emergency care and coverage for major medical. Claims are processed by a professional firm of administrators and paid out of the union's trust funds.

DENTAL and ORTHODONTIC

20% discount or no out of pocket expense (depending on your plan coverage, not exceeding the maximum plan benefit and provided you have coverage for the month of services.

For Union members without coverage, a 20% discount will be given off of the total visit charge.

CHIROPRACTIC and PODIATRY

Members and their families receive high-quality chiropractic (spine-related) and podiatry (foot care) either fully paid or at substantially reduced prices.

Many programs provide care at no cost to members and their families. In addition, orthodontic benefits keep out-of-pocket expenses to a minimum.

PRESCRIPTION DRUGS

Paid prescription coverage is our most-used benefit. Thousands of prescriptions are filled each month for a nominal service charge.

VISION CARE

Members and their families receive high-quality eye examinations and optical accessories either fully paid or at substantially reduced prices.

EMAP (Employee member Assistance Program)

United Food and Commercial Workers & Employers Arizona Health and Welfare Trust offers the "Employee Member Assistance Program (EMAP). It is a confidential, professional support service and referral program established by the Board of Trustees for eligible participants of the United Food and Commercial Workers & Employers Arizona Health and Welfare Trust. It is designed to assist you and your family members cope with a wide range of personal and work-related problems such as:

Living with a chronic illness can be challenging for both you and your family.
